Bilingual Clinical Pharmacist

📍 Tucson, AZ | 💵 $130,000–$140,000 + 10% Annual Bonus | 🕐 Full-Time | Mon–Fri, 8 AM–5 PM | Hybrid (4 days onsite, 1 day remote)

About the Role

We are seeking a Clinical Pharmacist to play a key role in enhancing the quality and affordability of care. In this position, you’ll provide medication management support, lead pharmacist-driven clinical programs, and collaborate closely with providers and patients to improve health outcomes. The focus will be on optimizing therapy, supporting chronic disease management (e.g., hypertension, diabetes, COPD, CHF), reducing preventable ED/hospital utilization, and ensuring medication adherence.

Pharmacists in this role meet patients where they are—virtually, in-office, or at home—to prevent, identify, and resolve medication-related issues. You will also contribute to clinical initiatives such as program development and quality improvement projects.

Responsibilities

  • Manage pharmacist consults for chronic care, post-discharge medication management, and curbside provider consults
  • Conduct comprehensive medication reviews using EMR, pharmacy data, and patient input; develop and follow up on care plans
  • Evaluate medication therapy for appropriateness, safety, effectiveness, and adherence challenges
  • Collaborate with providers, care teams, patients, and caregivers to establish disease-state therapy goals
  • Provide patient education on chronic conditions and prescribed therapies
  • Deliver services under collaborative practice agreements (refills, therapeutic interchange, disease management)
  • Act as a trusted drug information resource for patients, providers, and care teams
  • Participate in quality improvement programs and committee initiatives
  • Identify staff training opportunities to optimize patient care and the overall patient experience

Required Qualifications

  • Pharm.D. degree or equivalent education in pharmacy
  • Active, unrestricted Pharmacy license in Arizona (or ability to obtain)
  • Clinical pharmacy background (non-retail)
  • Bilingual in Spanish (high-level)
